Payment Agreement

Thank you for setting up a payment agreement!

Note that all payments setup via payment agreement will be run on the 15th of each month (or next business day if the 15th falls on a weekend or holiday).

Please indicate the MONTHLY payment amount in the field as indicated, as this will be your monthly payment for the duration of your payment plan. The duration will be based on balance due and your monthly payment amount.

NOTICE: Service Charges

SOME providers add a service charge to credit card transactions. The fees are noted below:

Payment Amount $1-250 = $3.00 service charge added

Payment Amount $251-500 = $5.00 service charge added

Payment Amount $501+ = $8.00 service charge added
